The American HealthCare Association provides savings to its members onmedical services through a number of medical networks. In order toaccess these networks and the related discounts, member or member'sdependents must pay the medical providers promptly. Payments on allmedical bills are due and payable at the time of service. The memberhas no out-of-network benefits and must use a contracted provider inorder to receive any benefits.

The member understands that the use of the program for cosmetic surgeryis extremely limited and can only be utilized by members whenpre-certification has been authorized and a referral number has beenissued to the patient.

As a service to members, the American HealthCare Association medicalsavings program may provide network rate information to medicalproviders under this program. If the information provided results in anunderpayment to a medical provider, member agrees to pay the medicalprovider for any shortages within ten (10) days of notice to suchmember of the inappropriate reimbursement. If the information providedresults in an overpayment to a medical provider, the AmericanHealthCare Association will assist member to the best of its ability tocollect any such amount from the appropriate party.

Neither the American HealthCare Association nor any of its affiliates,nor any network accessed shall be liable for any payment to a provideraccessed under the American HealthCare Association program, or anyrefusal of participating providers to accept the network rates offeredunder this program. The American HealthCare Association savings, itsaffiliates or any network accessed is not an insurer, guarantor orunderwriter of the responsibility or liability of Member for Member'sor Member's dependent's medical care or any other goods or servicesprovided to Member or Member's dependents.

Participating Medical Providers are independent contractors and, theAmerican HealthCare Association and its affiliates and its contractednetworks are not responsible for health care provided or the omissionof the provision of health care by any provider. The AmericanHealthCare Association does not practice medicine or in any mannerinterfere with or participate in the provider-patient relationship. Allhealth care decisions are between the patient and a provider. Theselection of a provider is the obligation and decision of the patientand is not based upon the credentialing or any recommendation by theAmerican HealthCare Association, its affiliates or its contractednetworks. The providers listed in this directory are subject to changewithout notice.

The American HealthCare Association medical savings reserves the rightto terminate any member for failure to pay a medical provider accessedunder the American HealthCare Association medical savings program underthe terms provided.

Members may cancel their American HealthCare Association medicalsavings program at any time upon written notice to the company andreturn of the ID cards.
